How to embed php into a page

I have a php form for hotel reservations here: http://lbsvilla.com/new/booking I need to add this to this page: http://lbsvilla.com/new/reservations I have not been able to do this using the embed widget as it shows the actual code, not the form.
How can this be done?

You can use the embed code module to embed an iframe to external application
